DNP STUDENT RESEARCH - OPPORTUNITY TO WIN A $100 AMAZON GIFT CARD

My name is Kayla Fornal and I am a Doctor of Nursing practice (DNP) graduate student in the College of Nursing at Florida State University. For my DNP project, I am examining nurse practitioner knowledge, attitudes, practice, and confidence regarding tongue and lip ties. If you are a nurse practitioner in Florida, please consider participating.
The following survey will require approximately 5-10 minutes to complete. It includes your demographic information, knowledge and current practices regarding tongue and lip ties. The educational module will require approximately 20 minutes to complete and can be paused and resumed at any time. After completing the educational module, you will take another survey that takes approximately 5-10 minutes to complete. One month after, I will email to repeat the survey one last time. Your total time for participation will not be greater than 1 hour.
Participation is entirely voluntary, and you may withdraw from this study at any time you wish. Survey completion serves as consent to participate. There is no compensation for responding nor is there any known risk. However, participants who complete the survey at all three time points (including the one-month follow up survey) will be entered to win a $100 Amazon gift card. Potential risks in this project include the risk of breach in confidentiality, every effort will be made to minimize this risk. There is no direct benefit, however there is potential benefit that includes receiving clinically significant information that could be used to influence the care provided to patients. There is also the societal benefit of increasing awareness to the phenomenon, identifying themes among the provider’s knowledge, attitude, current practices, and confidence in identifying and treating tongue-ties.
Should you choose to participate in this project, please answer the survey questions as honestly and completely as possible. In order to send the follow-up email and be entered for the gift card, please provide only your name and email address. This information will be separated from your survey responses and not linked in any way.
Thank you for taking time to assist me in my educational endeavors. The data collected will provide useful information regarding ways to improve patient care.
