Practice Setting
Post-acute medical facility- a healthcare setting that is designed to support patients who no longer require intensive hospital-level care, but still need medical oversight and rehabilitation to recover fully. This setting provides transitional care, which bridges the gap between hospital discharge and safe return home or to a lower level of care.
Multidisciplinary care is provided, including physical therapy (PT), occupational therapy (OT), and speech therapy (ST). Nurse practitioners and MDs provide oversight for individualized patient care and goal-oriented recovery, including supportive services such as dietary counseling and case management to assist with discharge planning needs.
The goal is to maximize recovery, reduce hospital readmissions, and promote independence.
Preceptorship Description
I am one of two FNP's - and approximately see 25-30 patients per day- of patients who present with a range of conditions. Such as rehabilitation after orthopedic surgery, rehabilitation of the brain, heart, and lungs. Care for chronic and complex medical conditions. No pediatrics. My schedule is 7 on/7 off.
- If you are an acute care NP student, you will gain experience from radiology reports, trending labs, and ordering transfusions.
- If you are a Family Medicine NP, you will learn to trend medication effectiveness, educate patients on secondary and tertiary interventions, manage pain, and offer family-centered care.
